PURPOSE:
The purpose of the Team Achievement Award is to recognize outstanding accomplishments in nutrition education with SNAP-eligible audiences. All achievements must be reproducible, have made a measurable impact, and use best practice standards. More favorable consideration will be given to those projects performed through diverse collaborative partnerships and those focusing on the SNAP initiatives to promote healthier eating behaviors, such as increased calcium and fruit and vegitable consumption and reducing nutrition-related diseases, such as obesity, diabetes, heart disease and cancer.
DIVERSE COLLABORATIVE PARTNERSHIPS:
Collaborative partnerships are encouraged between FNS programs, such as: SNAP-Ed Program, EFNEP, WIC, USDA, and TEFAP; health care organizations such as: Horizon Mercy and Amerigroup; voluntary health agencies, such as: The American Cancer Society and 5-a-Day; community-based organizations and faith-based organizations, as well as corporate and trade organizations.
NOMINATION CRITERIA:
- Teams working in New Jersey, from the following groups, are eligible:
- SNAP-Ed Support Network Members
- SNAP Members
- Individuals or teams may be nominated by colleagues or may self-nominate.
- Accomplishments in nutrition education programming will be considered in the selection of award recipients.
SELECTION PROCESS:
Completed applications must be submitted online by 3:00PM on October 31, 2006. The winning team will be announced at the February 28, 2007 Statewide Network meeting. All applications should be submitted via the e-mail application form available by clicking "Nominate a Team! Click Here" below. Photos and other supporting materials are welcomed as part of your application. Project descriptions should be limited to 1,000 words. Awards will be distributed at the Statewide Network meeting on April 25, 2007. At that time we would like
to request that the winning team present their project.
Award:
Recipients will be honored at a luncheon held at Rutgers, The State University of New Jersey.
